I'm a resident of Ontario or Quebec (health care is governed by jurisdiction and Dr. Dana can only provide therapy to residents of Ontario or Quebec).

I'm between the ages of 18 - 60 years. Dr. Dana is only licensed to provide therapy to this age group.

I'm available between the hours of 9 am - 5 pm Monday - Friday.
